The Oklahoma Palliative Care Resource Center presents: ADVANCE HEALTH CARE PLANNING

This seminar explains the importance of planning in advance for a serious illness in the event that you can no longer make your own health care decisions.  Participants will complete the Oklahoma Advance Directive form, including a living will and appointment of a health care agent.  The speaker will be Annette Prince, JD, LCSW, MA Bioethics & Health Policy, Assistant Professor Oklahoma University Health Sciences Center.  (Bring a pen or pencil to complete the form.)
